Vaginismus

What is Vaginismus?

Vaginismus (now often classified under genito-pelvic pain/penetration disorder) is an involuntary tightening of the pelvic floor muscles, which can make penetration painful or impossible. This can affect sexual intercourse, medical exams, or tampon use. It may result from physical or psychological factors, and effective treatment combines physical therapy, gradual desensitization, and counseling.

Causes

  • Physical: Infections, injuries, hormonal changes, menopause, childbirth complications

  • Psychological: Fear of pain, anxiety, past sexual trauma or abuse, stress, relationship difficulties

Symptoms

  • pain, burning, or tightness during attempted penetration.

  • Difficulty inserting tampons or during pelvic exams

  • Muscle spasms in the vaginal area

Treatment Options

Treatment often combines physical and psychological approaches:

  • Pelvic floor physical therapy to relax and strengthen muscles

  • Cognitive-behavioral therapy (CBT) or counseling to address fears and anxiety

  • Gradual desensitization using vaginal dilators

  • Open communication with partners and healthcare providers to support recovery
