HRT for Menopause: Is Hormone Replacement Therapy Safe?

Hormone Replacement Therapy (HRT) can be an effective option for some people experiencing troublesome menopause symptoms. However, it is not automatically appropriate for everyone. Understanding the different types of HRT, their potential benefits and risks can help you have an informed conversation with a healthcare provider.

1. What is HRT?

Hormone Replacement Therapy replaces hormones that the body produces less of during menopause, primarily oestrogen and, when needed, progestogen.

It can be given in different forms, including tablets, patches, gels and other preparations.

2. What symptoms can HRT help with?

Systemic HRT can be particularly effective for symptoms such as hot flushes and night sweats. It may also help with other menopausal symptoms.

When vaginal symptoms such as dryness or discomfort are the main concern, local vaginal treatments may be more appropriate than systemic HRT.

3. Does everyone experiencing menopause need HRT?

No. Some people experience few symptoms and do not need treatment, while others have symptoms that significantly affect their quality of life.

4. Is HRT safe?

5. Why might someone need both oestrogen and progestogen?

For someone who has a uterus, taking systemic oestrogen alone can stimulate the lining of the uterus and increase the risk of endometrial cancer.

A progestogen is generally used alongside oestrogen to protect the uterine lining. People who have had a hysterectomy may have different treatment requirements.

6. What is the difference between systemic and local HRT?

Systemic HRT delivers hormones into the bloodstream and can help manage symptoms affecting the body more broadly, such as hot flushes.

Local vaginal oestrogen delivers a low dose directly to vaginal tissues and is primarily used for vaginal and some urinary symptoms.

The appropriate treatment depends on the symptoms and individual circumstances.

7. Can HRT help with vaginal dryness and painful sex?

Hormonal treatment can help some menopausal vaginal symptoms.

For vaginal dryness and pain associated with menopause, a healthcare provider may recommend local vaginal oestrogen or another treatment, depending on the individual's health history and symptoms.

8. Should I start HRT without speaking to a healthcare provider?

No. HRT should be individualised.

A qualified healthcare provider can review your symptoms, medical history, medications and individual risk factors and discuss the available options with you.
